There is no cost to register. The process will take about 2 minutes. You will have access to the Phzio Health Portal to book appointments with your Health Care Team and access on-demand health content.
Provide your personal information, benefit program and provincial health card details. You will receive an email with a link to complete your functional physical along with the requisitions needed for your ECG and lab tests.
Once we get your labs and functional results back, you will work with the Care Coordinator to: Review your labs with the Nurse Practitioner, review functional results with the Athletic Therapist and discuss dietary needs with the Dietitian to design a comprehensive plan to enable you to reach your health goals.
You will receive a welcome email with an attachment for your lab requisitions and ECG requisition. You will take this to a local lab for specimen collection and ECG testing. The lab will send us your results.
In your welcome email you will also have a link to perform your functional tests for Strength, Cardio, Balance, Flexibility and Posture. You can perform these movements when it works for your schedule.
Once your labs, ECG and functional testing is complete you and your care coordinator will schedule sessions to review your results and create a plan. This includes sessions with a Nurse Practitioner, Athletic Therapist and Dietitian.
