Register
There is no cost to register. The process will take about 5 minutes. You will have access to the Phzio Health Portal to book appointments with your health care team.
Set your Health Priorities
Complete the health priorities questionnaire to inform your team and set the baseline of your goals. The more information you provide the better your Health Team can ensure you have the plan you want.
Start the Program
You will need to complete some personal information including your provincial health card/program information. You will receive an email with a link to complete your functional physical (do this at your own speed) as well as the lab requisitions you will need to get your ECG and Labs completed.
Provider Sessions
Once we get your labs and functional results back, you will work with the Care Coordinator to review your labs with the Nurse Practitioner, review functional results with the Athletic Therapist and discuss dietary needs with the Dietitian to design a comprehensive plan to enable you to reach your health goals.

- Kick-off call with your Care Coordinator (Registered Practical Nurse)

- Electrocardiogram requisition to test various cardiac factors

- Video reviews and one-on-one follow-ups with each Health Provider
- A Plan of Care for abnormal results and lifestyle recommendations
- Specialist referrals where clinically indicated.
